Who is helping indigent seniors in their homes with meals and daily living? Who is taking care of those who can’t take care of themselves? Who is providing hospice services for those who are in need?
Thanks to the Philadelphia Foundation’s Delaware County Health Care Assistance Fund, DCCF’s Home Health Care Fund, with the help of local non-profit service providers, were able to accomplish the following in 2016.
- Provide over 215 indigent senior citizens with 4 weeks of Hot meals delivered to their door.
- Provided over 260 patients with over 9,800 days of telemonitoring service which reports vital signs to nurses thus improving and saving lives.
- Provided over 727 indigent and low income terminally ill Delco residents within-home hospice care so they could live our their final days in peace at their homes.
- Provided over 300 Indigent Home Bound patients with properly skilled nursing, physical therapy, home health aides and more.
The Home Health Care fund, supported by The Philadelphia Foundations’ Delaware County Home Care Assistance Fund has supported organizations who service these individuals. Our goal is to provide these much needed services to all who are in need.
